Abstract

Alveolar pulmonary proteinosis is the accumulation of surfactant in the alveolar spaces and terminal bronchioles, evidenced by the dysfunction of the alveolar macrophages, which causes respiratory failure. It is not very prevalent in people of all races, sexes and ages. The worldwide prevalence is up to 7 cases per million inhabitants, at the moment without local epidemiology. This is the case of a young woman with a history of alveolar proteinosis who presented exacerbation secondary to a confirmed Sars-Cov 2 infection, so she required lung lavage. She was readmitted for repeated spontaneous pneumothoraces and is awaiting lung transplantation. This is one of the few published cases of coincidence of both pathologies and in the current post-pandemic context, it is expected that these cases will be presented more frequently, so it is important to determine guidelines for proper diagnosis and management.
