Abstract
Mesenteric cysts are intra-abdominal tumors with a rare presentation, it is estimated that approximately 1 in every 250 000 hospitalizations in adults, their symptoms are usually secondary to the mass effect since they are generally 10cm in diameter. In this report an adult who came to the emergency department in the context of abdominal pain, absence of depositions, emesis and hyporexia so a tomographic study was performed that showed an extrinsic compression of the distal ileum suspicious of an ovarian cyst with indication of surgical management. What leads us to carry out this report is the size of the cyst, which was documented in 19.5x17x11cm and a weight of 20.15kg, which makes it an atypical presentation that represents a challenge diagnosis and surgical skills in the moment of resection.
