Abstract

Peritoneal carcinomatosis is a rare entity with a poor prognosis, evidenced by metastases mainly of gastrointestinal and ovarian origin. However, it is rare for it to develop because of prostatic adenocarcinoma. We present the case of an older adult with constitutional syndrome associated with malignant ascites of two months of evolution. The tomography revea- led multiple peritoneal lesions corresponding to peritoneal carcinomatosis. On rectal examination, fixed, stony prostate with bilateral recesses involved. Markedly elevated prostate specific antigen. Biopsy of peritoneal lesions confirmed ad- vanced prostatic adenocarcinoma, T4N0M1, stage IVb, Gleason X; Complete androgen blockade was performed, and he continues to be followed up at 24 months. Peritoneal carcinomatosis due to prostate carcinoma is a differential diagnosis that must be considered in the presence of ascites and prostate tumor pathology, with clinical suspicion and cytochemical studies being the beginning of the sequential approach for a timely diagnosis, tumor staging and prognosis.
