Diagnóstico prenatal del genotipo Rhd fetal

Resumen

RESUMEN 

La isoinmunización RhD puede causar enfermedad hemolítica del feto y el recién nacido si el tipo de sangre del feto es RhD positivo y el de la madre RhD negativo. A pesar de que la incidencia de isoinmunización severa ha disminuido con el uso profiláctico de inmunoglobulina anti-D durante la gestación y después del parto, la sensibilización aún ocurre en un pequeño grupo de mujeres. En dicho grupo la enfermedad por Rh continúa siendo un problema importante que puede comprometer la salud y la vida del feto o neonato. Identificar tempranamente los fetos en riesgo permite establecer el plan de seguimiento para determinar el momento de la intervención y el uso adecuado de recursos. La presente revisión tiene como fin presentar los avances actuales en diagnóstico prenatal del genotipo RhD y analizar su posible implementación como prueba de rutina.

ABSTRACT 

RhD isoimmunization can cause hemolytic disease of the fetus and newborn if the fetus blood type is RhD positive and the mother's is RhD negative. Even though the incidence of severe isoimmunization has decreased with the use of prophylactic anti-D immunoglobulin during pregnancy and after delivery, sensitization still occurs in a small group of women. In such group Rh disease continúes to be a significant problem which can jeopardize the fetus or neonate's health and life. Early identification of the fetuses at risk allows the establishment of a follow-up plan to determine the moment of intervention and an adequate management of resources. The present revision presents the latest advances in prenatal diagnosis of fetal RhD genotype and analyzes its possible implementation as a routine test.
